Upon receipt and acceptance of your application, the following will be required:

• Two passport photos
• Your martial arts resume
• Copies of your most recent and/or highest rank promotion Certificates
• A one-time membership fee of U.S.$100.00
• Checks are to be made payable to Otto Johnson

I understand that by becoming a Wadokai I agree to host individually or collectively one Wadokan seminar per year in my immediate area at a cost of U.S.$500.00. Travel expenses, lodging and transportation for the Dojocho and/or one or more additional assistant instructors, if needed, to and from the seminar(s), independent of the seminar fee, shall be the responsibility of the hosting Wadokai.

It is also understood that I may register my current rank and/or any future rank promotions for either my students or myself with the Wadokan at the rates so stated in the Wadokan Rank Promotion Fee Schedule.

By this form, I hereby agree to the terms stated in this membership application, and understand that failure to comply with them shall result in the termination of my Wadokai membership.
